This is a sample clinical supervision agreement form. It will allow an opportunity for you to:
 
Clearly define what supervision is
Describe the structure of supervision (frequency, fees & payments, no-show/late cancellation policies, etc.)
Supervisee expectations  & responsibilities
Supervisor agreements & commitments
Supervisor values and scope of practice
 
Heads up: A supervision agreement is not a requirement for the states I am licensed to provide supervision in (FL & SC). If you are interested in using this template and are a supervisor in a state where a clinical supervision contract is required, it is your responsibility to ensure that the agreement you and your supervisee sign meets the needs and professional obligations of your state and your state's board.
